Not all individuals are eligible for the Yellow Line Project. Attached are our current offense categories.
Our screener completes this form in the pre-booking area or in the community and reviews charges to determine if the individual is eligible for diversion. This covers the four areas of concern: medical, mental and chemical health, and corrections. This form helps the CBC and individual develop a plan.

A Release of Information form is signed to coordinate services to entities for each individual. *NOTE: This form is printed on carbon copy paper to prtect orivacy while only requiring one signature from the indiviual.

The individual, law enforcement officer, and screener must all sign the agreement to start the diversion plan. This is what we refer to as "the three yes's."
An individual in our jurisdication must sign and comply with the 12 conditions of the Yellow Line Participation Standards.
This form is an explanation of privacy rights. Information will only be shared if allowed.
If an YLP individual is leaving the pre-booking location, a release of liability form is signed by the person taking them off the premise. The receiving person is agreeing to accept liability for the individual who was detained.
When an individual completes their My Yellow Line Plan, a success letter is sent to the officer involved in the original incident.
In the event that an individual does not meet expectations and/or complete their My Yellow Line Plan, a letter is sent to the officer recommending the individual to be criminally charged.
