Operational Toolkit: Forms, policies and procedures were created to support the needs of the community based coordinator doing post-arrest jail diversions. Click on an item on the list below, or scroll down further for additional information about each item.

The purpose of any release of information form is to coordinate services for the individual. This form marks what information the individual is allowing to be shared with a human services, health care providers, mental or chemical  health care facility, and/or a treatment center.

The individual, law enforcement officer, and screener must all sign the agreement to start the diversion plan. This is what we refer to as "the three yes's."


An individual in our jurisdication must sign and comply with the 12 conditions of the Yellow Line Participation Standards.

If an YLP individual is leaving the pre-booking location, a release of liability form is signed by the person taking them off the premise. The receiving person is agreeing to accept liability for the individual who was detained.


When an individual completes their My Yellow Line Plan, a success letter is sent to the officer involved in the original incident.

In the event that an individual does not meet expectations and/or complete their My Yellow Line Plan, a letter is sent to the officer recommending the individual to be criminally charged.
